Just get form 10A100-FI(6-05) from the Commonwealth of Kentucky, Department of Revenue, P.O. Box 299, Station 20, Frankfort, KY 40602-0299, (502) 564-3306, or go to:
www.revenue.ky.gov and download the form. You will have to file for a few months on a monthly basis and then they go to a semi-annual basis.
